2. Apply by 5/3/2024 for Employee Guarantee Issue*
*Benefit Eligible Employees working 20+ hours per week are eligible to apply. Participation requirement of 5% approved employee applications needed to offer Guaranteed Issue. All employees will answer underwriting questions in their application – if participation requirement is met, application will be processed with Guaranteed Issue and the underwriting questions will be disregarded. Guarantee Issue up to $100,000 for employees ages 19-70. Other policy options are available with Underwriting consideration. Spouses require Underwriting consideration for all levels of coverage.
